Your first visit:
For your first visit, please plan to arrive 15 minutes early to provide information about your medical history. If the appointment is for your child, you will be required to fill out a form for him or her, as well. For your convenience, you may download our Health History Form in Microsoft Word format below. You may bring the form with you to our office or fax it to us prior to your appointment. Our fax number is: 865-546-3797. We also have our Privacy Notice in the same format for your review.
Payment:
We accept most major dental insurance. As a courtesy to our patients, we will complete and submit the necessary paperwork to your insurance provider. Should you have any questions regarding your coverage, please contact your carrier directly, as it is impossible for us to know the specifics of your particular policy.
We also accept cash, checks, and most major credit cards. We offer payment plans and options at very affordable rates, including CareCredit which meets a variety of financial needs with up to 24 months at 0% interest.
If you have a Flexible Spending Account set up with your employer, and you are able to plan your dental treatment ahead, you can pay pretax dollars toward your dental costs to help cover portions not covered by your insurance. For instance if you elect $1,000 for the year and are in a 28% tax bracket, you will only really pay $720 - so saving $280! Talk to your employer.
Parking:
Patient parking is available in the Medical Arts Building garage, or there is surface parking adjacent to the building.
